MV-912 is the New York DMV's bill of sale for a private vehicle sale. New York asks for proof of ownership (the signed title or transferable registration) and proof of the purchase price when the buyer registers the vehicle, and MV-912 covers the price. It also gives both parties a dated record of who sold what to whom.

What the MV-912 is for, and who fills it in
What it is for. Documenting the sale price and the transfer for the buyer's registration and title application, and for the sales tax the buyer pays at registration.
Who fills it in. The seller and the buyer together. Both sign. If the vehicle is a gift, the parties also complete form DTF-802 for sales tax purposes.
Where it goes. The buyer takes it to a DMV office with the title, the registration application (MV-82), sales tax form DTF-802, proof of identity and insurance. The seller keeps a copy and removes the plates.
The fields that matter
| Field | What to enter |
|---|---|
| Vehicle | Year, make, model, body type, color, and the full VIN, copied from the title. If the vehicle is registered, the plate number. |
| Sale | The sale price in dollars and the date of sale. |
| Seller | Seller's printed name, address and signature. Each owner shown on the title signs. |
| Buyer | Buyer's printed name, address and signature. |
| Odometer and damage disclosure | Not on MV-912. New York's odometer and damage disclosure statements are completed on the back of the title certificate (or on form MV-103 if there is no space). |
How to fill out the MV-912 in FormFill
- Get the official PDF onto your phone. Tap the agency link above and save the PDF (on iPhone, Share, then Save to Files; on Android it lands in Downloads). Only use the agency's own copy: the form changes, and an old revision can be rejected.
- Import it into FormFill. Open FormFill and tap Import PDF, then pick the file. FormFill writes on top of the page, so it does not matter whether the PDF has fillable fields or is a flat scan.
- Fill the fields. Tap Text, then tap where an answer belongs and type it. Drag to move, pull a corner to resize. Use Check for the checkboxes and Date where a date is asked for. Pinch to zoom on the small boxes.
- Sign it. Tap Sign to draw your signature (or reuse a saved one with Pro) and place it on the signature line, not on the printed-name line. Initials covers any initial boxes.
- Finish and send. Tap Done. FormFill flattens your entries into the PDF and offers Share a copy or Save to device. Open the exported file once before you send it.
On this form: Type the vehicle details from the title, the price and the date, then have each party add their Sign signature. The seller and buyer can each keep the finished PDF; the buyer prints it for the DMV office.
Mistakes to avoid
- The sale price on MV-912 should match the price on DTF-802, the sales tax form the buyer also files.
- New York requires the seller to remove the plates; the plates belong to the seller, not the vehicle.
- The odometer reading goes on the title's disclosure statement, which the seller must complete for most vehicles.
- A vehicle with a lien needs the lien release as well; the bill of sale does not clear it.
