Printable ABC chart template
A blank ABC data collection sheet with the columns behavior analysts and IEP teams expect: date, time, setting, antecedent, behavior, consequence, intensity and duration. Print it for the fridge, the classroom or a sitter. When the sheet is full, type the rows into the online ABC log or the Nello app and the counting is done for you.

Intensity: 1 brief and mild · 2 upset but settled quickly · 3 clear meltdown or shutdown · 4 intense, needed an adult · 5 unsafe or very long. Antecedent is what a camera would have shown, not the cause. Behavior is what you saw, not a label. Consequence means what happened after, including what helped. Using the sheet
- One sheet per place. Home and school happen under different conditions; keep them on separate sheets with the setting filled in, then combine them in the summary.
- Write within the hour. Antecedents are the first thing memory drops. A row written at bedtime about a 7 a.m. moment is usually missing the "just before".
- Minutes matter. A rough number is better than a blank. Ten incidents of two minutes and two incidents of forty minutes tell very different stories at the same frequency.
- Don't skip the quiet days. A week with one row is data too; it is the baseline every later week is read against.
- Two to three weeks of consistent rows is usually enough for the first pattern to show. The FBA summary page shows what the counts look like once they are totalled.
Frequently asked questions
What columns should an ABC chart have?
Date and time, setting, antecedent, behavior, consequence, and usually an intensity rating and a duration in minutes. Those last two turn a diary into data that can be counted.
How many rows per page?
12 by default, enough for most weeks at home. Choose 8 for more writing room, or 16 for a classroom aide who logs many short incidents.
Can I get it as a PDF?
Tap Print and choose Save as PDF in the print dialog; the print layout removes everything but the sheet.

Nello captures the same row on your phone in under a minute, with photos, and builds the FBA-ready ABC data report (PDF + CSV) from the whole record. A teacher can add the school day from a link, no app or account needed.
