ABC data sheet: a printable ABC chart and an online ABC log
Record a hard moment as Antecedent, Behavior and Consequence. Print the blank sheet for home, school or a sitter, or log here and get the counts a BCBA, school psychologist or IEP team asks for: frequency per day, duration, time of day, and what came before and after. Made by the team behind Nello, the autism behavior tracker.
Online ABC log
Rows stay in this browser until you export them or save them into Nello. Three example rows are loaded so you can see the summary; clear them when you start.
Summary
Time of day
Counts by weekday and part of the day. The same grid is in the Nello app's PDF report.
Export
The CSV has the same columns as the app's nello-abc-data.csv: date, time, weekday, setting, antecedent, behavior, consequence, intensity, duration.
Save to the Nello app
In Nello: Export & reports → School-day log link → Make a teacher link, and open that link in this browser (it works for you as well as for a teacher). The rows in this log are then added to that person's record on every phone that shares it, and the app builds the full PDF report from there. Nothing from the record is shown on this page.
This browser is not connected yet. Make the link in the app and open it here within seven days; it works once and then this browser stays connected, add-only, until you remove it in the app.
This browser is connected to a Nello record.

How to fill in an ABC data sheet
AAntecedent
What was happening in the minute or two before. Not the cause, just the setting of the scene: a request, a transition, a sound, a wait, hunger, a change of plans. Write what a camera would have shown.
BBehavior
What you observed, in words anyone could check: "dropped to the floor and screamed for about ten minutes", not "had a tantrum" or "was defiant". Length and intensity go in their own columns.
CConsequence
What happened right after: what adults did, what the child got or avoided, and what helped. "Consequence" here means what followed, not a punishment.
Two or three weeks of consistent rows is usually enough for the first pattern to show. Our guide to ABC data collection at home covers common mistakes, and the worked examples show filled-in rows for school, home and transitions.
Frequently asked questions
What is an ABC data sheet?
A chart with one row per incident and columns for the Antecedent (what happened just before), the Behavior (what was observed, in plain words) and the Consequence (what happened right after, including what helped). Most sheets add the date, time, setting, intensity and duration. It is the standard way parents, teachers and behavior analysts collect data for a functional behavior assessment (FBA).
Is this ABC chart template free?
Yes. The printable sheet, the online log and the summary are free with no account. The Nello app is free to use too; its PDF ABC data report is part of the optional Nello+ upgrade.
Where does the data I type here go?
Nowhere, unless you choose to. Rows live in this browser only. When this browser is connected through a Nello log link and you tap Save to Nello, those rows are sent over an encrypted connection to the Adelyons sync server so the family's phones can fetch them. This page never reads anything back from the record.
Is this ABA?
ABC recording is a general observation method used across many approaches, not a therapy in itself. Nello stays neutral on therapy modalities: it helps you record what happened and what helped, and leaves interpretation and any plan to the professionals you work with.
What does "FBA-ready" mean?
A functional behavior assessment starts from counts: how often, how long, when in the day and week, in which settings, after which antecedents and followed by which consequences. The summary here, and the PDF report in the Nello app, lay those out as tables so nobody is re-counting your notes by hand. It is data, not an assessment.
The same ABC capture, in your pocket
Nello logs a meltdown as ABC in under a minute, with the setting, intensity, duration and photos, and builds the FBA-ready ABC data report (PDF + CSV) from the whole record. A family keeps one shared record across phones, and a school-day log link lets a teacher add the school hours with no app and no account. Local-first, works offline.
