Fatigue with pelvic pain or periods

A tiredness that sleep does not fix, that arrives with the pain days and takes a day or two to lift. People with endometriosis report it constantly, and it rarely makes it into the notes because it does not sound like a gynaecological symptom. In the self-check it is recorded as context: it does not count toward the six NICE groups, and it belongs on the sheet.

Not a diagnosis. This page explains one symptom group that published guidance says should prompt a clinician to consider endometriosis. No symptom, score or checklist can tell you whether you have it; only a clinician can assess you. If pain is severe or sudden, seek medical help.

What the guidelines say

NICE NG73's list of reasons to suspect endometriosis does not include fatigue; it concerns pain, bowel and bladder symptoms and infertility. The Office on Women's Health does list fatigue among the symptoms people with endometriosis experience, and ACOG describes the impact on daily life as part of the assessment. That is why Wrenly asks about it and prints it under "what to tell your doctor" rather than counting it as a guideline feature.

How to describe it at an appointment

What to track before you go

Wrenly logs energy as "spoons" (0 to 5) every day next to pain, and Wrenly+ shows whether flares cluster on your low-energy days. The doctor PDF prints the energy column by date.

Check your whole pattern

This symptom is one of 14 questions in the free endometriosis symptom self-check. It scores your answers against the six NICE symptom groups and ends in a one-page prep sheet you can print for your doctor. Nothing you enter leaves your browser.

Frequently asked questions

Is fatigue a symptom of endometriosis?

It is widely reported by people with endometriosis and listed by the Office on Women's Health, though NICE does not use it as a criterion for suspecting the condition. Raise it alongside the pain; on its own it has many causes worth checking.

Why does the self-check not count fatigue toward the score?

The score counts the six symptom groups NICE NG73 lists. Fatigue is not one of them, so counting it would overstate the match. It still appears on your prep sheet as a point to raise.
